Attendees who need visa for travel to the Netherlands should complete the following form and send email to
- your full name (first and last names),
- affiliation,
- passport number,
- place of issue (passport),
- date of birth,
- gender,
- place and country of birth,
- consulate/embassy where you will apply for visa.
- Please note that invitation letters will be issued only to registered participants of the conference who have already paid the conference fee.
- If you have family members traveling with you, please provide also their information as mentioned above.
